I think the key thing is that as soon as the interviewer stops, you (the candidate) need to continue onwards with the case unless they pull you another way. To be totally mentally alert and full of beans, I woke up particularly early before my BCG final to arrive 30 mins before and basically wander around town on the way in with a coffee.
